Applications for information about a minor/incompetent patient
When the application is on behalf of a minor, the natural guardians (parents) are required to:
* Sign the attached application.
* Present/send a photocopy of their ID card, including the appendix in which the minor is recorded.
In the event of an incompetent patient, the guardian is required to send a detailed application signed by the legal guardian according to the court order.
The guardianship order and ID card/photocopy of the ID card should be sent by Mail to gs@laniado.org.il upon provision of the information. Issuing the information is subject to a fee. The recipient will sign receipt of the information.
Laniado Hospital was founded in 1976 by the late Rabbi Yekusiel Yehuda Halberstam, Sanz Klausenberg Rebbe, who lost his wife, eleven children and many relatives in the Holocaust.
