Information applications from an attorney
The attorney will attach the following:
- Detailed application
- A medical information confidentiality waiver containing a witness to the signature
- A power of attorney
The forms should be stamped with an “True Copy” stamp
Issuing the information is subject to a fee. The attorney will receive a payment demand and once the payment has been transferred, the information will be sent by registered mail or by messenger on the attorney’s behalf, who will provide us with a power of attorney from the attorney.
