Applications for information about a deceased patient
By law, transfer of information after the death of a patient is permitted by presentation of:
- An inheritance order/probate order/affidavit of heirs.
- ID card of the heir contacting the Medical Records Department
Issuing the information is subject to a fee.
Upon provision of the information, recipient will sign receipt of the information.
Laniado Hospital was founded in 1976 by the late Rabbi Yekusiel Yehuda Halberstam, Sanz Klausenberg Rebbe, who lost his wife, eleven children and many relatives in the Holocaust.
