Information applications by anyone on the patient’s behalf
Anyone seeking a copy of a medical record or part thereof should contact the Medical Records Department or send an application by Mail to gs@laniado.org.il together with a medical confidentiality waiver signed by the patient and an identity document of the patient and the applicant.
Issuing the information is subject to a fee (may be paid over the phone by credit card). Upon provision of the information, the party arriving on behalf of the patient will sign receipt of the information.
Laniado Hospital was founded in 1976 by the late Rabbi Yekusiel Yehuda Halberstam, Sanz Klausenberg Rebbe, who lost his wife, eleven children and many relatives in the Holocaust. He remained alone to deal with the unbelievable suffering of that period on his own. Many miracles and amazing tales are told about the Rabbi’s path in the Holocaust.
