Information applications after discharge
Anyone seeking a copy of their medical record or part thereof should contact the Medical Records Department by mail or email to gs@laniado.org.il after completing and attaching a patient information application form together with photocopy of an identification document. Issuing the information is subject to a fee. The information may be received by email once payment has been made by credit card over the phone.
Laniado Hospital was founded in 1976 by the late Rabbi Yekusiel Yehuda Halberstam, Sanz Klausenberg Rebbe, who lost his wife, eleven children and many relatives in the Holocaust. He remained alone to deal with the unbelievable suffering of that period on his own. Many miracles and amazing tales are told about the Rabbi’s path in the Holocaust.
